Care Coordinator VillageCareMAX 112 Charles Street, New York, NY 10014 Hourly Rate: $25.01 - $28.14 Must Be Bilingual - Spanish, Chinese, Korean VillageCareMAX is looking for a motivated individual for this Full-Time role to provide assistance to care managers and members in coordinating services and requests. The Care Coordinator will work to authorize services based on plan of care and ensure that services are initiated and delivered. Some of your daily activities will include: Respond to inquiries from members and providers regarding health insurance, benefits, eligibility and authorization. Initiate contact with care management team, provider relations department and external providers to meet member's needs. Document, track and follow up with care management team for all member service requests. Perform monthly member contacts and documentation in the electronic health record. Maintain and support documentation, logs and data entry to monitor activity and/or outcomes related to Medicare billable services. Reports any member dissatisfaction and grievance as appropriate Participates in special projects and perform other duties as assigned We would like to speak to those who have 3+ years job related experience. High school diploma required. Excellent communication skills, organizational skills and problem solving will be vital to this position. Bilingual skills highly required.
