Open-Minded Therapy, LCSW, PLLC is a rapidly growing mental health practice committed to providing accessible, high-quality care to individuals and families. Our diverse team of clinicians and support staff is passionate about making a positive impact in the lives of our clients. As we expand our reach, we are looking for a compassionate, highly organized Care Coordinator to serve as a key point of contact for clients and support them through the coordination of their care.

Role Description

The Care Coordinator plays a vital role in ensuring clients have a seamless and supported experience as they engage in services. This position requires excellent communication skills, attention to detail, and the ability to balance administrative responsibilities with client-centered coordination. The ideal candidate is professional, personable, and thrives in a fast-paced, mission-driven environment

Responsibilities:
• Coordinate client care and follow up on services
• Communicate with clients regarding scheduling, intake, and next steps
• Ensure treatment plans and required documents are completed and signed
• Assist with referrals and coordination across providers
• Monitor client engagement and follow up on missed appointments
• Maintain accurate and timely documentation in the EHR
• Support clinicians with coordination and administrative needs

Qualifications:
• Bachelor’s degree in Social Work, Human Services, or related field preferred
• Experience in behavioral health, care coordination, or healthcare settings preferred
• Strong organizational skills and attention to detail
• Ability to manage multiple tasks and follow through independently
• Strong communication and interpersonal skills
• Comfortable working in a remote environment
• Experience with EHR systems is a plus
