Bilingual RN Care Manager (Remote Flexible - Spanish Speaking)
Pair Team is seeking a full-time Registered Nurse Care Manager to support patient-driven care plans and improve outcomes for individuals with Serious Mental Illness, Substance Use Disorder, homelessness, and/or high medical needs. The role involves working in a team-based model with other care managers and nurse practitioners to provide health education, complex care management, medication reconciliation, care coordination, and clinical interventions. The position is primarily remote (about 90% work from home) with 1-2 on-site community visits per month. Responsibilities include managing a caseload of individuals with complex medical needs, collaborating with multidisciplinary teams, conducting assessments and screenings, and building trust with underserved populations. Candidates must hold an active California RN license, be bilingual in English and Spanish, have experience in care coordination or case management, and possess strong technical skills. The role requires a quiet, HIPAA-compliant home work environment and reliable internet and phone access.
