We are seeking Advanced Practice Providers (Nurse Practitioners) to support our health plan partners by delivering highâquality, virtual preventive health assessments.
We offer:
Flexible, selfâdirected schedules
Scheduling available 7 days/week, including excellent weekend options
1:1 patient interactions focused on wholeâperson health
Assessmentâonly model (no prescribing or treatment changes)
Preâpopulated clinical templates for efficient documentation
Competitive, marketâbased pay
NY state license required
Compensation & Earning Potential:
Earn up to $2,400 per week (at max volume)
Competitive base rate: $120â$150 per completed assessment
Progressive bonus structure: Increase earnings as volume grows, TierâoverâTier
Topâtier potential: Earn over $10,000 monthly
In addition to this flexible feeâbased model, youâll have the opportunity to support meaningful health outcomes, improve access to care, and work independently with the full support of a dedicated clinical team.
What You Will Do â Essential Functions
As an InâHome Assessment Provider, you will perform comprehensive virtual health risk assessments (HRAs) for Medicare, Medicaid, and ACA members. These assessments help identify care needs, support early intervention, and improve care coordination.
Clinical Care & Assessment Delivery
Perform virtual health assessments, including medical history reviews, physical exams, screenings, and medication reviews.
Conduct thorough evaluations of chronic and acute conditions.
Utilize preâpopulated clinical templates to support efficient, accurate documentation.
Submit chart notes within required timeframes and address queries promptly.
Maintain clear communication with clinical leadership and operations teams.
Scheduling & Member Engagement
Provide a selfâmanaged schedule with a minimum monthly availability expectation (varies by market).
Offer appointment availability across days, evenings, and weekends.
Deliver empathetic, patientâcentered care in a respectful and professional manner.
Educate members on wholeâperson wellness and connect them to appropriate resources.
Who You Are
Required Qualifications:
Active, unrestricted NP license in New York state.
Board certification (ANCC, AANP, or NCCPA).
New York candidates: Also accepting board-certified or board-eligible MDs.
Current BLS certification.
Ability to pass all background checksâincluding criminal, SSN validation, education, employment verification, healthcare sanctions, sex offender registry, and drug screen.
Strong clinical assessment and diagnostic skills.
Proficiency with EMR systems and comfort using digital tools.

Must have strong computer skills, and both knowledge and proficiency with electronic medical records.
Have a strong understanding of risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA).
Demonstrate empathy and respect for members while completing in-home assessments
Working conditions:
Mental and physical stamina to perform assessments and documentation.
